Retrospectives should never be used to criticize individuals or place blame. While your team is going through the forming-storming stages—and thereafter when necessary—it's helpful to start off by displaying some version of the Retrospective Prime Directive, such as this one:

Regardless of what we discover today, we believe that everyone did the best job he or she could, given the level of knowledge at the time, the skills and abilities at play, availablethe resources available, and the situation at hand. Not only does everyone make mistakes, and  Everybody makes mistakes. And on occasion, we repeat the same mistakes.
