In Photoshop CS5:

  • Open image in Photoshop.
  • Crop or resize to 359 x 192
  • Layers Panel: Click on base layer and Layer from Background (Layer 0)
  • Layers Panel: Click on base layer and Duplicate Layer (Layer 0 copy)
  • Layers Panel: Create a new layer (Layer 1). You should now have 3 layers in this order from top to bottom: Duplicate Layer (Layer 0 copy), Background Layer (Layer 0), New Layer (Layer 1)
  • Click on Layer 1 in the Layers Panel. Next click the Image menu from the main Photoshop menu bar and choose Canvas Size from the drop-down. Change the units to pixels and change the values to something like 370 x 240. In the anchor area, click the top-middle arrow. Make sure that if you can choose, that the background color be transparent.
  • Click on Layer 0 in the Layers Panel and click Command-t on your keyboard. You should see an outline around your image. In the menu bar at the top of Photoshop, there should now be an icon that looks like an arc that says "switch between free transform and warp modes" when you hover over it. Click that, and choose Arc Lower from the dropdown and type in the value -5 into the Bend field. Hit Enter.
  • In the Layers Panel, double click on Layer 0 and check Drop Shadow in the Layer Style pop-up. Change the Opacity to 54%, the Angle to 90, Distance = 8, Spread = 0, Size = 5. Click OK.
  • In the Layers Panel, double click on Layer 1 and check Stroke in the Layer Style pop-up. Change the Size to 1px, the Position to Inside and the Color to 999999. Click OK.
  • Next click Image from the main Photoshop menu bar and choose Trim.
  • Finally, save for web and devices as a transparent png.
